In June 2019, UNICEF convened a group of about 40 global experts from 24 organizations and institutions to advance the collective thinking on paediatric HIV service delivery. The aim of this “think tank” consultation was to build consensus on the specific programme interventions that need to be scaled up to improve the quality of HIV treatment services and reach more infants, children and adolescents with these lifesaving medicines.

The paediatric service delivery framework presents strategies to address bottlenecks across the continuum of care for each population: infants, children and adolescents. This includes keeping mothers who receive interventions for the prevention of mother-to-child HIV transmission (PMTCT) and their infants in care; locating missing infants, children and adolescents through family and index testing; linking those diagnosed with HIV to services; treating them with efficacious regimens and retaining them on treatment to achieve viral suppression. It describes comprehensive and targeted service delivery models, which emphasize strong linkages between testing, treatment and care, and between communities and facilities.

The framework was developed by a group of global experts who were convened by UNICEF in June 2019 to advance the collective thinking on paediatric HIV service delivery. The partnership's analysis of current evidence and specific programme interventions that need to be scaled up to improve the quality of HIV treatment services and reach more infants, children and adolescents with these lifesaving medicines is presented here.

An extension of Key Considerations for Introducing New HIV Point of Care Diagnostic Technologies in National Health Systems, this Toolkit contains various practical tools and guidance to support countries as they introduce point-of-care (POC) HIV technologies into existing national diagnostic networks and laboratory systems.

Organized using a modular framework, this Toolkit covers the following building blocks for HIV point-of-care testing:

Site and Product Selection
Site Enrollment and Training
Site Monitoring and Post-Market Surveillance
Quantification and Supply Planning
Quality Assurance

Importantly, both the Toolkit and the Key Considerations guidance were developed in the context of pioneering, catalytic investments made by Unitaid in new point-of- care HIV diagnostic technologies across 15 countries in Sub-Saharan Africa: Cameroon, Côte d’Ivoire, Democratic Republic of the Congo, Ethiopia, Kenya, Lesotho, Malawi, Mozambique, Rwanda, Senegal, Swaziland, Tanzania, Uganda, Zambia, and Zimbabwe. Robust implementation experience across more than 10 countries has served as the foundation for the development of both resources.

At the turn of the century, and the beginning of the Millennium Development Goals, an HIV diagnosis was equivalent to a death sentence for most children and their families in low-income countries. But now, an early diagnosis paired with treatment and care can ensure long healthy lives, regardless of location, and can help prevent transmission of HIV to others. Since 2000, thirty million new infections were prevented, nearly eight million deaths averted, and fifteen million people living with HIV are now receiving treatment.

“It would be one of those histories where you’d say, ‘A miracle happened’,” said Sheila Tlou, Director of UNAIDS, Eastern and Southern Africa Region.

Here is a glance at a slice of that miracle, and a look toward the unfinished business.

EMTCT of HIV & Syphilis in Thailand (Mar 2017)

A total of 30 participants from India, Nepal, Nigeria, Sri Lanka, Thailand and the United States attended the webinar on March 20, 2017 to discuss Thailand's lessons learned in reaching the elimination of mother-to-child transmission (EMTCT) of HIV and syphilis. Thailand is the first country in the Asia and the Pacific region and the first with a large HIV epidemic to receive validation from the WHO for achieving this milestone.
