Adolescents

The AIDS movement, led by people living with and affected by HIV, continues to inspire the world and offer a model for a people-centred, rights-based approach to global health and social transformation. And yet, today, amid a swirl of competing and complex global concerns, we confront a serious new obstacle: the oppressive weight of complacency. This is happening when we know that if we make the right decisions and the right investments now, the end of AIDS can be within our grasp. This moment is, however, fleeting. We have a fragile window of opportunity—measured in months—in which to scale up.

This document was created by ICAP’s Clinical and Training Unit with valuable input from our teams in Swaziland, Mozambique, Kenya, and Cote d’Ivoire. It was developed as a template document to be adapted for use in various contexts and is one component of a viral load monitoring tool-kit, to be used in conjunction with ICAP’s Viral Load Monitoring Flipchart and Enhanced Adherence Treatment Plan. This area is evolving rapidly therefore it is expected that this document will require frequent updating over time, as recommendations change, and needs to be adapted according to local guidelines and context.

Monitoring of individuals on ART is important to ensure treatment efficacy and improved health outcomes. Updated WHO Consolidated ARV Guidelines will be available in December 2015 and include recommendations on routine monitoring and the diagnosis of treatment failure.

Adolescents face significant barriers accessing and remaining in HIV treatment and care services. WHO guidelines in 2015 support initiation of antiretroviral therapy for all adolescents living with HIV. Adolescent friendly health services should be developed, including support for adherence and retention in care.

The 2016 High-Level Meeting on Ending AIDS will focus the world’s attention on the importance of a Fast-Track approach to the AIDS response over the next five years. The UNAIDS Fast-Track approach aims to achieve ambitious targets by 2020, including:

1. Fewer than 500 000 people newly infected with HIV
2. Fewer than 500,000 people dying from AIDS-related causes
3. Elimination of HIV-related discrimination

This third edition of the Malawi Guidelines for Clinical Management of HIV in Children and Adults will be implemented starting May 2016. It replaces all previous editions of the Malawi Antiretroviral therapy (ART) and Prevention of Mother to Child Transmission (PMTCT) guidelines. This document is written for medical doctors, clinical officers, medical assistants, nurses, midwives, health surveillance assistants (HSAs) and medical records clerks who are working in public and private sector health facilities in Malawi. It is designed to be a practical guide for implementation of integrated HIV Services.

The new consolidated guidelines provide standardized, simplified and less toxic drug combinations harmonized for the management of Prevention of Mother to Child Transmission (PMTCT), children, adolescents and adults with HIV/AIDS, TB and other common opportunistic infections. It will provide guidance for clinicians, managers and trainers on the use of available regimens within the context of continuum of HIV comprehensive care for prevention, treatment and support for all age groups in private and public sector to realize our vision of a long and healthy life for all citizens. This approach will also ensure that people living with HIV are started on the right regimen at the right time.

Em Moçambique, a pandemia do HIV/SIDA é assumida como um dos desafios de âmbito multissectorial. No que concerne ao Serviço Nacional de Saúde, múltiplas acções integradas têm sido implementadas nos últimos anos pelo Ministério da Saúde (MISAU) com vista a alargar as medidas de prevenção, diagnóstico e o início atempado dos cuidados e tratamento, sendo que em 2013 foi implementado o Plano de Aceleração de Resposta ao HIV e SIDA, que contempla nos seus objectivos a expansão dos Serviços TARV e a melhoria da qualidade assistencial.

This handbook has been written for practitioners working directly with children and adolescents living with HIV and for policy makers and management to help develop services and protocols. To that end, it includes policy and practice guidance for the development of services, practice models and practical examples. The global experience of HIV offers many shared elements and this handbook reflects these, setting out practical guidance and tools that can be used in different settings.

This report guides countries through the process of collecting and reporting sub-national data on adolescents to inform programme planning and implementation efforts. This guidance has been developed with the specific aim of identifying data gaps for adolescents and informing immediate programme planning needs at the sub-national level.
